Competition Entry Form 

Introduction 

The substance of this entry form should be prepared by the entrant (with assistance for compilation purposes if required). The judges will use the presented entry only to make their decisions (which are final). The form is a web form to enable easy addition of text and photographs! Please try to keep within the size limitations and do not include any photographs showing people, only models and parts etc.  You can download to save the final version as a pdf document. Photos are required to illustrate some questions, and these should be added where requested, with a clear title for each photo. Please crop or reduce photos but make sure that they are still clear. Supporting adults only should provide the requested contact details in case of questions for them. No addresses are required. Please ensure that all boxes are completed: Adult contact information will only be used, if needed, for confirmatory purposes and communication regarding the entry to the competition. If the entry is selected for a prize, then the entrant with parents/guardians (if under 18 years old) will be invited to receive the prize at the FMES AGM event. At this point, parents/guardians of winners under 18 years old will be asked to provide a release form to allow photographs to be taken at this event for publicity purposes.
